SimplyFind PT organizes publicly available records from the National Plan and Provider Enumeration System (NPPES) to make provider information easier to search. Public records can change, so every directory record should be treated as a starting point for verification.

Our source data
NPPES is a federal provider-enumeration system administered through the Centers for Medicare & Medicaid Services. The records are publicly disseminated under the CMS data-dissemination program and can be searched through the NPI Registry. Read CMS data-dissemination information and search the NPI Registry.
We refresh public provider-record information on a recurring schedule. A directory record may still change between refreshes, including practice location, contact information, taxonomy details, or organization relationships.
What an NPI does—and does not—show
An NPI is a provider identifier. By itself, it does not prove current state licensure, credentials, appointment availability, insurance acceptance, quality, suitability, or a current relationship with a particular practice. Visitors should verify those details with the relevant official source, practice, and health plan as applicable.
How to request a correction
If you see a possible issue, use our corrections process. Include the provider name, NPI if available, page link, and a concise explanation. Where the source record itself is inaccurate, providers should also update NPPES directly where applicable.
